Your First Appointment

Please arrive 15 minutes before your appointment time

Before your first appointment:

  • Log into your patient portal before your first appointment to add your medical history/medications and sign your registration form
  • Wear or bring clothing the right clothing:
    • Women should bring/wear a tank or camisole under their clothing for neck/shoulder conditions
    • Bring/wear shorts for low back, hip, knee or foot conditions
  • Bring a photo ID, a form of payment, and your insurance card if applicable
  • We recommend you call your insurance company prior to your arrival to verify your physical therapy coverage and requirements

Frequently Asked Questions

Do I need a Referral?

No, a referral is not required to see a therapist in the state of Minnesota. However, your insurance may require one to cover the charges.

Do you participate with my insurance?

We participate with most major payers, including Medica-Mayo Clinic Employees (Tier 1), BCBS, UHC, Medica, Aetna, Medicare, Medicaid, UCare, Preferred One, Humana, and many more).

Some insurance plans have special network restrictions. We recommend you call the phone number on the back of your insurance card to confirm your specific plan’s coverage for physical therapy services before your first visit.

What questions should I ask my insurance company?
  • Is ActivePT in-network with my insurance? If you are told we are “out of network,” call us to obtain our Tax ID Number for easier look-up by your insurance plan.
  • Do I need a doctor’s prescription/referral for physical therapy?
  • What is my deductible? Is it met yet?
  • After my deductible is met, what is my copay or co-insurance for each visit?
  • How many physical therapy visits are covered?
  • Is prior authorization required for physical therapy?
What if I'm not using insurance?
If you are not using insurance, are uninsured or if ActivePT is an out of network provider, you may choose to use our Pay at the Time Of Service fee schedule. When you pay, in-full at the time of service qualifies for a 30% discount off our standard fees. We accept Visa, Master Card, American Express and Discover Card, cash, and check as forms of payment.
What are the typical costs?

If you would like to receive more information about the cost of your therapy, we recommend you meet with one of our physical therapists for a free screening appointment. This helps us better understand your condition, your goals, the total course of your care. Knowing this information helps us provide you a better estimate of costs.

  • Our first visit ranges from an estimated $170 to $320, depending upon the complexity of your condition
  • We offer direct billing to your insurance
  • Financial hardship may qualify you for additional discounts, ask for more details
